CARE Navigator is an innovative intervention for young people and their families who are involved or at-risk for involvement in the juvenile justice system.
The program aims to identify and address challenges which may lead young people to become court-involved including mental health, substance use, and trauma-related disorders. We help families successfully navigate the justice system, connect with social services and resources, and find a pathway to success.
Utilizing the Family SBIRT model (Screening, Brief Intervention, and Referral To...), we work with families to identify areas of need, develop a comprehensive service plan, connect them with appropriate social services and resources, and offer ongoing support and service navigation. Families are paired with a Navigator -- an individual who provides families with ongoing support in order to improve access to services and empower them to achieve sustainable success. CARE Navigator is offered through a formal partnerships with the 29th Judicial District Court, the St. Charles Parish Sheriff’s Office, and the St. Charles Parish Public Schools.
